The worry didn\u2019t come from thin air. After big endurance efforts, some runners show temporary changes on echocardiograms, and some have a spike in a blood marker called troponin\u2014a protein released into the bloodstream when heart muscle cells are stressed or injured, and one doctors use when they\u2019re assessing possible heart attacks. A new long-term follow-up study, published in <a href=\"https:\/\/jamanetwork.com\/journals\/jamacardiology\/article-abstract\/\"><em>JAMA Cardiology<\/em><\/a>, finally tested the part about whether those finish-line blips turn into lasting damage years down the road.<\/p>\n<p><img decoding=\"async\" class=\"alignnone wp-image-152767 size-large lazyload\" data-src=\"https:\/\/runningmagazine.ca\/wp-content\/uploads\/2025\/12\/runners-heart-resized-to-1200x800-1-1024x683.jpeg\" alt=\"runner's heart\" width=\"788\" height=\"526\" data-srcset=\"https:\/\/runningmagazine.ca\/wp-content\/uploads\/2025\/12\/runners-heart-resized-to-1200x800-1-1024x683.jpeg 1024w, https:\/\/runningmagazine.ca\/wp-content\/uploads\/2025\/12\/runners-heart-resized-to-1200x800-1-300x200.jpeg 300w, https:\/\/runningmagazine.ca\/wp-content\/uploads\/2025\/12\/runners-heart-resized-to-1200x800-1-768x512.jpeg 768w, https:\/\/runningmagazine.ca\/wp-content\/uploads\/2025\/12\/runners-heart-resized-to-1200x800-1.jpeg 1200w\" data-sizes=\"(max-width: 788px) 100vw, 788px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 788px; --smush-placeholder-aspect-ratio: 788\/526;\" \/><\/p>\n<h2>What the new study tracked<\/h2>\n<p>Researchers followed 152 male recreational marathon runners (mean age 43) and measured heart function and biomarkers before the race, immediately after, at one day and three days, and again at 10 years.<\/p>\n<h2>What happened to the right ventricle<\/h2>\n<p>Right after the marathon, something called <em>right-ventricular ejection fraction<\/em> (RVEF) dropped\u2014the finding that has fuelled concern. The median value went from 52.4 per cent pre-race to 47.6 per cent immediately after finishing. By the next day, it had improved to 50.7 per cent, and by day three it was essentially back (51.3 per cent). Ten years later, it was still stable at 51.9 per cent. The study found no meaningful association between the post-race troponin response and right- or left-ventricular ejection fraction 10 years later. That refutes the idea that a common <a href=\"https:\/\/runningmagazine.ca\/health-nutrition\/your-marathon-brain-could-be-making-you-sick\/\">post-marathon<\/a> troponin bump is a warning sign of future ventricular damage, at least in runners like the ones studied.<\/p>\n<h2>Small changes on the left side stayed within normal ranges<\/h2>\n<p>Left ventricular measures shifted slightly in ways that were statistically significant, but the values stayed within normal physiological ranges. It\u2019s a reminder that \u201cchanged\u201d doesn\u2019t necessarly mean \u201cdamaged.\u201d<\/p>\n<h2>The takeaway<\/h2>\n<p>This is the kind of long follow-up runners have been missing.
